We didn't qualify, so we got the pity discount (my name for it) of about $2,000. We have a $2000 copay each month, but after the first month, we go into the catastrophic coverage category, then the copay drops to $823 (or something like that.) The financial team at my cancer center called me to get the necessary info and then filed for us. Your cancer folks ought to be able to help with this, or, as I understand it, the AZ website itself has application information. Eligibility is based on income and you have to reapply yearly. I'm retired and on Medicare and have been eligible for the last 2 years.
